The Liberia National Police acknowledge the tragic death of N-tom-bee-keh-yee-seh Innocentia Khumalo, commonly known as “Toni“, a South African national. We extend our deepest condolences to her family, loved ones, and the people of South Africa. This is a moment that demands not only empathy, but clarity, discipline, and fidelity to the rule of law.
From the moment this matter was brought to the attention of the Liberia National Police, a full-scale, multi-disciplinary investigation was initiated.
This investigation was conducted by:
The Crime Against Person Unit, The Forensic Section, and In coordination with medical authorities and the Montserrado County Coroner.
Every step taken has been guided by:
Evidence, Professional standards, and the legal framework of the Republic of Liberia.
KEY FINDINGS OF THE INVESTIGATION
The investigation has established a body of evidence that cannot be ignored.
Pattern of Prior Violence: There is credible and corroborated evidence indicating that the victim was subjected to repeated physical violence over time, supported by: Witness accounts, Photographic evidence, and Documented communications.
Controlled Environment and Last Contact The defendant, Samuel P. Jackson, was:
The last known person with the victim inside a secured residence and was also the first person who entered the secured room and discovered the victim unconscious and in a pool of blood, where both entry and exit were restricted, effectively isolating the victim.
Forensic and Medical Evidence (CRITICAL) Medical and forensic findings now establish a clear and compelling trauma profile: Contusions to: Left forehead, Back of the head, and Middle and right chest wall; Laceration and hemorrhage to the back of the head; Bruising on the upper limbs with pinpoint puncture marks, suggestive of possible restraint or defensive injury.
Trauma Sequence (Causation)
The injuries support a probable sequence of events: A blunt force impact to the chest caused a backward fall, resulting in severe head trauma, including contusion, laceration, and hemorrhage. This is not a random or isolated injury, as It is a linked a chain of previous traumatic occurrences.
Neglect and Duty of Care
Evidence further establishes that: The victim had a known medical vulnerability which occasionally resulted in seizures, yet the suspect continued to supply the stimulant. The victim was left the lone in a locked environment, without supervision. This constitutes a serious breach of duty of care, with foreseeable risk to life.
Post-Incident Conduct
The investigation also identified: Failure to promptly notify law enforcement, and Indicators that the scene was altered prior to police arrival. These actions are legally significant and form part of the evidentiary record.
FORENSIC STATUS (ONGOING SCIENTIFIC PROCESS)
A full autopsy report will further clarify the extent of internal injuries. Toxicology analysis is ongoing, with results expected within 2 weeks, or possibly sooner.
CRIMINAL CHARGES
Based on the totality of evidence, the Liberia National Police has formally charged Mr. Samuel P. Jackson with: Manslaughter, Negligent Homicide, Aggravated Assault, Reckless Endangerment of Another Person
In violation of Chapter 14, sub-chapters “A &B, Sections 2, 3, 20, and 23 of the Revised Penal Code of the Republic of Liberia. The matter will be immediately forwarded to the court for adjudication.
